Medical declaration for using tanning equipment
Applies to sunbed tanning services — MegaSun Beauty House, Timișoara.
This is a self-declaration. By booking, the client confirms that they have read, understood, and accepted the information below. Use of the tanning equipment is at the client's own risk.
1. Minimum age
Sunbed tanning services are intended exclusively for persons 18 years of age or older. By using the service, the client confirms having reached this age.
2. I confirm I do NOT have any of the following conditions
- ✖Known photosensitivity
- ✖Treatment with photosensitizing medication (e.g. antibiotics, retinoids, antidepressants)
- ✖Pregnancy
- ✖Active skin conditions
- ✖History of skin cancer
- ✖Recent sunburn
- ✖Recent use of depilatory creams or skin peels
3. Recommendations
Protective goggles are mandatory. Sessions should not exceed 20 minutes, and a minimum 24-hour interval between sessions is recommended. If in doubt about your health, consult a doctor before use.
4. Acknowledgment
By checking the 18+ box on the booking form, I confirm having read and understood this declaration and accept responsibility for using the equipment.
